Are you a U.S. citizen who is age 16 or older and needs to apply for a passport with a DS-11 form?
Are you the parent or guardian of a U.S. citizen who is age 15 or younger and needs to apply for your child’s passport with a DS-11 form?
Do you need to renew an existing passport using the DS-82 form?
Do you need to report a passport lost or stolen and replace it using the DS-11 and DS-64 forms?
Do you need to correct or change a passport that you already have using the DS-5504 form?
Do you need to apply for a passport for a child under age 16 who cannot have both parents or guardians apply in person?

You must provide one photo with your passport application. All our photo policies apply to both adults and children under age 16. We have a few exceptions to our photo policies for infants.
